Cancellation Policy

We require 48 hours cancellation notice for any missed appointments. Please call us; do not send email.

We reserve the right to charge a cancellation fee for any appointments missed without proper notice.

FEES & INSURANCE

We request that all fees are paid at the time service is rendered, including co-payments. If your treatment requires extensive work, a payment schedule may be arranged. We also accept Visa, MasterCard and Discover for your convenience.

Dental Insurance

We are a participating provider with Delta Dental only. For all other plans we are considered out-of-network, which means that your benefit levels and co-payments could be affected, so please check with your insurance for coverage details.
